We are seeking a highly skilled Home Health Physical Therapist to join our team at LHC Group. As a leading provider of post-acute care, we are committed to delivering high-quality, cost-effective services that support our patients when and where they need it.

About the Job

The successful candidate will be responsible for assessing and evaluating patient care needs related to functional status/mobility, endurance/stamina, pain and other medical conditions. Based on this assessment and evaluation, the Physical Therapist will work to determine a treatment plan, perform interventions aimed at improving and enhancing the patient's well-being, and evaluate the patient's progress.

Responsibilities
  • Assess and evaluate patient care needs related to functional status/mobility, endurance/stamina, pain and other medical conditions.
  • Determine a treatment plan based on the assessment and evaluation.
  • Perform interventions aimed at improving and enhancing the patient's well-being.
  • Evaluate the patient's progress and make adjustments to the treatment plan as needed.
Requirements

To be considered for this role, you must have:

  • A degree in physical therapy from an accredited program.
  • A current license to practice physical therapy in the state of Connecticut.
  • Current CPR certification.
  • A reliable vehicle and valid driver's license (or access to public transportation).

LHC Group offers a competitive salary range of $83,500 - $123,000 per year, depending on experience. Additionally, we offer:
