About the Role

We are seeking a compassionate and skilled Certified Nursing Assistant to join our team at Hospice of the Midwest. As a member of our team, you will play a vital role in providing exceptional patient care and support to individuals and their families in their homes.

Responsibilities
  • Provide personal care services to patients, including bathing, dressing, and grooming
  • Assist with daily living activities, such as meal preparation and medication reminders
  • Monitor and report changes in patient condition to the Case Manager
  • Provide emotional support and companionship to patients and their families
  • Collaborate with the hospice team to develop and implement individualized care plans
Requirements
  • Meets the training requirements of the State
  • Completed a minimum of seventy five hours of classroom and supervised practical training
  • Minimum of six months experience in providing personal care
  • Ability to read and follow written instructions and document care given
  • Understands hospice philosophy and is comfortable providing specialized care to the terminally ill
