Physical Therapist Home Health Full Time

We are seeking a skilled Physical Therapist to join our Home Health team. As a Physical Therapist, you will play a vital role in helping patients recover from injuries, illnesses, and functional disabilities, enabling them to regain their independence and live healthier lives.
Key Responsibilities- Plan and administer prescribed skilled physical therapy treatment and training for patients
- Test and screen patients' physical strengths to assist physicians in evaluating their level of function
- Develop and implement conditioning and rehabilitation programs consistent with physicians' plans of treatment
- Confer with physicians and clinical team members to obtain additional patient information and assist in developing, implementing, and revising therapy treatment programs
- Provide written instructions and care plans to Physical Therapy Assistants and Home Health Aides
- Degree from an accredited Physical Therapy Program
- Minimum of one year physical therapy experience required
- Home Health experience preferred (OASIS documentation a plus)
- Current and unrestricted Physical Therapy license
- Current CPR certification
