  • POSITION OVERVIEW
    • The Advanced Practice Nurse collaborates with medical professionals to deliver comprehensive healthcare services by distinguishing between standard medical findings and those necessitating further evaluation or referral to a physician. The Advanced Practice Nurse assesses and tracks health conditions, executes diagnostic and therapeutic procedures, and provides age-appropriate education and counseling focused on disease prevention, management, risk mitigation, and health upkeep. This role also involves adhering to established medical protocols for addressing minor acute illnesses and chronic health issues, prescribing medications within the authorized scope of practice, and coordinating referrals for specialized care or social service assistance. Additionally, the position supports population health initiatives through wellness visits, transitional care management, and advanced care planning.
  • ESSENTIAL QUALIFICATIONS
    • Education: Must be a Registered Nurse with advanced training through a program designed to prepare for the role of an Advanced Practice Nurse.
    • Experience: Prior experience in an outpatient or primary care environment is required.
    • Licensure/Certification/Registration: Must hold a valid license to practice in the State of Wisconsin and be certified by a national nursing organization as a Certified Nurse Practitioner in the areas of adult, family, pediatric, or OB/GYN care. BLS certification is also required, with a designation of either BLS Provider or Healthcare Provider from recognized organizations. A Wisconsin Background Study is mandatory upon hiring and must be renewed as per state regulations.

    • Ability to provide care tailored to patients of all ages and to work collaboratively with physicians to ensure a seamless continuum of care.
    • Strong interpersonal skills to foster harmonious working relationships with colleagues and patients. Proficient in educating patients and families about health promotion and disease prevention strategies.
    • Effective communication skills to interact with patients, family members, physicians, support staff, and other healthcare professionals.
    • READING - Intermediate: Capable of interpreting documents such as operating instructions and procedural manuals.
    • WRITING - Advanced: Proficient in composing articles and speeches that adhere to established style guidelines.
    • SPEAKING - Intermediate: Able to present information effectively in various settings, including one-on-one and group situations.
    • MATHEMATICAL SKILLS - Intermediate: Competent in basic arithmetic and able to perform calculations involving percentages, ratios, and other mathematical concepts.

  • PHYSICAL REQUIREMENTS AND WORK ENVIRONMENT
    • PHYSICAL DEMANDS
      • Must be able to meet the physical demands of a primary care role, including the ability to see, hear, and communicate effectively, as well as engage in prolonged standing, walking, reaching, stooping, bending, kneeling, and crouching. The role may require lifting objects up to fifty (50) pounds and working shifts that may exceed eight (8) hours but are less than twelve (12) hours.
    • LIFTING REQUIREMENTS
      • Ability to lift up to 10 pounds continuously, up to 25 pounds frequently, and up to 35 pounds occasionally.
