This position is part of our Skilled Nursing Facility (SNF) Patient Connect program. Optum Senior Community Care offers the Patient Connect Program for United Healthcare members in transitional care settings, focusing on reducing 30-day hospital readmissions, enhancing the accuracy of diagnosis and medical record documentation, increasing closure rates of STAR/HEDIS quality measures, improving completion rates of Advanced Care Planning Directives, and elevating patient and family satisfaction along with discharge and post-discharge support. The Optum practitioner will conduct a comprehensive health assessment for members in collaboration with the case management team to facilitate better care coordination and health outcomes. This role operates autonomously, providing significant satisfaction for the Nurse Practitioner as they influence the care and support of our population.
This is a per diem role. Per diem employees are expected to be available for a minimum of 10 hours per week, with a maximum of 960 hours annually. A flexible schedule is available, including days, evenings, and weekends.

Key Responsibilities:

Essential Qualifications:
- Active and unrestricted license in your state of residence
- Certified Nurse Practitioner through a recognized national board
- For NPs: Graduate of an accredited Master's degree in Nursing (MSN) program and board certified through the American Academy of Nurse Practitioners (AANP) or the American Nurses Credentialing Center (ANCC), with preferred certifications in Adult-Gerontology Acute Care Nurse Practitioners (AG AC NP), Adult/Family, or Gerontology Nurse Practitioners (ACNP), preferably certified as ANP, FNP, or GNP
- Current active DEA licensure/prescriptive authority or ability to obtain post-hire, per state regulations
- Access to reliable transportation for travel to SNF facilities within a designated area
- Ability to lift a 30-pound bag and navigate various dwelling conditions
- Availability to work a minimum of 10 hours per week
- Ability to establish a collaborative practice agreement, if applicable in your state
Preferred Qualifications:
- 1+ years of hands-on post-graduate experience in Long Term Care
- Knowledge of Geriatrics and Chronic Illness
- Familiarity with Advanced Illness and end-of-life discussions
- Proficient computer skills, including the ability to document medical information in both written and electronic formats
- Demonstrated ability to foster and maintain positive customer relationships
- Proven adaptability to change
Note for Washington Residents: The hourly range for Washington residents is $40.87 to $80.43. Compensation is based on various factors, including education, work experience, and certifications.
