The Outreach Community Health Worker will maintain a presence in the community to promote and improve healthy living. This role involves assisting individuals in accessing resources and services to overcome barriers to achieving their health goals.
Key Responsibilities- Make direct contact with community members to address barriers to access and the importance of preventive and comprehensive healthcare.
- Travel to various locations to meet community members where they are, such as their homes or community organizations.
- Maintain strong relationships with these organizations to ensure effective support for community members.
- Conduct needs assessments using the Social Determinants of Health Tool to identify potential barriers to accessing primary care services.
- Help community members access resources for social issues like homelessness, rental assistance, utility assistance, food, transportation, childcare services, substance abuse, and other issues impacting their lives.
- Schedule appointments for community members with primary care providers or specialists.
- Identify community members' insurance eligibility and assist with renewing their insurance online.
- Enroll community members in the CHW program for 30 days and conduct weekly follow-up calls or home visits.
