Lead Patient Care Manager RN

We are seeking a compassionate and experienced Registered Nurse to join our team as a Patient Care Manager RN at Heart of Hospice, LLC. As a key member of our clinical leadership team, you will play a vital role in ensuring the delivery of high-quality patient care and promoting a culture of excellence within our organization.

Key Responsibilities:
  • Lead and motivate a team of healthcare professionals to provide exceptional patient care
  • Ensure all staff are oriented and supported to meet their job performance goals
  • Liaise between staff, patients, and the medical community to ensure seamless care coordination
  • Assist the Executive Administrator/Executive Director/Director of Clinical Operations with patient care activities
  • Comply with hospice policies, procedures, and regulatory requirements
  • Actively promote quality of care and patient safety
Requirements:
  • Current RN licensure in the state of practice
  • One year of clinical experience
  • Current CPR Certification
  • Valid driver's license, vehicle insurance, and access to a dependable vehicle or public transportation
