PRIMARY ROLE:
Deliver nursing support to individuals aged 16 and above requiring Skilled Long-Term and Short-Term Care, including those who are chronically ill, elderly, or infirmed. The main objective of your role is to provide routine daily nursing and rehabilitative care in accordance with the resident's assessment and care plan, under the supervision of a licensed nurse. As a Certified Nursing Assistant, you play a crucial role in executing your assigned responsibilities. While this job description aims to be comprehensive, it does not limit the duties you may be required to perform. The absence of specific tasks does not exclude them from your role if they are similar, related, or logically assigned. You will report to the Nursing Supervisor or Unit Director.
RESPONSIBILITIES:
PERSONNEL DUTIES:
- Adhere to work assignments and schedules in completing tasks.
- Execute all tasks in line with established policies and procedures as directed by the
- Receive nursing reports upon arrival and provide updates to the Charge Nurse before concluding
- Display photo identification at all times.
- Document all entries on flow sheets, nutrition records, elimination logs, activity logs, rehabilitation records, and complete all documentation electronically or as instructed.
- Notify the RN or LPN Charge Nurse of any changes in the resident's condition promptly.
- Report any accidents or incidents observed during your shift as they occur, providing a statement for each.
- Engage in performance assessment and improvement activities to enhance the quality
NURSING CARE DUTIES:
- Provide routine resident care, which includes:
- Bathing
- Oral Hygiene
- Hair Care
- Nail Care
- Personal Hygiene
- Assistance with Toileting
- Feeding Support
- Assistance with Mobility
- Promote resident comfort through:
- Proper positioning, lifting, and transfers
- Mobility support
- Responding to call lights
- Addressing emotional needs
- Monitor and report any signs of pressure sores or skin breakdowns to prevent decubitus ulcers.
- Assist with post-mortem care.
- Measure and record weights, temperature, pulse, respiration, and blood pressure as directed.
- Respond to all call bells/lights promptly.
- Ensure adequate resident nutrition by participating in meal service and assisting residents with feeding.
- Serve and assist residents with snacks and nourishments as assigned.
- Collect and transport specimens as instructed.
- Assist in the care of residents in isolation.
- Perform prescribed rehabilitation exercises, range of motion activities, and utilize adaptive equipment.
- Fold and deliver residents' personal clothing.
- Assist in executing the Resident Assessment.
- Participate in Care Plan meetings to develop and review Resident Care Plans.
- Provide nursing care according to the established Care Plan.
- Suggest modifications to the resident's Care Plan and goals based on observations and
SAFETY AND SANITATION:
- Operate all equipment safely after receiving training on its use.
- Report any defective equipment to the Unit Director/Charge Nurse immediately, label it, and inform the Unit Clerk to request repairs.
- Responsible for cleaning resident equipment such as commode chairs, wheelchairs, shower chairs, and geri-chairs.
- Adhere to policies and procedures related to Standard Precautions.
- Maintain cleanliness in residents' rooms, dresser drawers, closets, utility rooms, shower/tub rooms, and corridors.
- Comply with state requirements for PPD testing.
- Follow emergency protocols related to Fire/Disaster procedures.
- Attend continuing education programs designed to enhance knowledge of caring for residents
- Participate in DNV and OSHA mandated training annually.
- Promote and support the mission and philosophy of the Facility during job duties and community activities.
- Actively engage in Continuous Quality Improvement initiatives.
- Perform other related duties as required.
- Maintain confidentiality of all resident care information.
- Uphold the rights of all residents to independent expression, decision-making, personal freedom, dignity, and protection from abuse or social isolation.
- Ensure privacy for residents by knocking before entering their room and closing doors or curtains before performing procedures.
- Report residents' complaints or grievances to the Charge Nurse as soon as possible.
- Attend all annual mandatory educational programs as required by the position.
- Understand and demonstrate the importance of meeting the needs of the customer/patient by interacting in a friendly and caring manner, being attentive to their psychological and physical needs, and maintaining communication to provide a secure and pleasant experience.

EDUCATION:
High school diploma preferred.
EXPERIENCE:
Prior experience or job knowledge is required. Nursing aide certification is mandatory. If hired without certification, must complete the requirements for nurse aide certification for job retention within a specified timeframe.
CPR REQUIREMENTS:
Heartsaver CPR certification required within a specified timeframe of hire. Basic Life Support (BLS) certification required within a specified timeframe of hire.
PHYSICAL DEMANDS:
- Good physical and mental health is necessary due to the demands of standing and walking for extended periods.
- Manual dexterity is essential for handling instruments and equipment.
- Visual and auditory acuity are required to make adequate observations of residents' conditions.
- Demonstrate understanding, patience, and tact in interactions with residents and colleagues.
- Adhere to and support Hospital policies and standards.
- Trustworthy in safeguarding confidential information.
- Willingness to work hours that best meet the needs of the service.
- Maintain a neat and professional appearance at all times.
- This position requires limited access to medical records, which includes charting on flow sheets, ADL, and graphic.
OTHER REQUIREMENTS:
The certified nurse aide must be listed as current in the Nurse Aide Registry, with verification of the CNA's current certification status, and no findings or convictions of resident abuse, mistreatment, neglect, or misappropriation of resident property listed on the Registry. The printed verification from the Registry confirming all the above must be filed in the CNA's personnel records.
