About the Role

Key Responsibilities
  • Coordinate care across the continuum, collaborating with multidisciplinary teams to ensure timely and effective transitions.
  • Develop and implement individualized care plans, taking into account patient needs, preferences, and cultural backgrounds.
  • Communicate effectively with patients, families, and healthcare providers to ensure clear understanding and alignment of care goals.
  • Identify and address potential barriers to care, developing strategies to mitigate risks and promote patient safety.
  • Stay up-to-date with regulatory requirements, industry best practices, and emerging trends in care coordination and transition management.
Requirements
  • At least 4 years of experience in pediatric nursing, case management, care management, care coordination, utilization review, or community-based nursing.
  • BSN required; Accredited Case Manager (ACM) or Certified Case Manager (CCM) or Care Coordination and Transition Management (CCTM) certification preferred.
  • Registered Nurse in the State of Texas upon hire.
