Our clinicians visit the homes of our patients, providing a continuum of care for individuals with chronic conditions and limited mobility.
We offer a range of programs, including remote patient monitoring, behavioral health management, and chronic care management, to ensure our patients receive the best possible care from a team they trust.
Purpose
We are seeking an experienced Advanced Provider to join our team on a full-time basis, providing primary care to our patients in and around Frederick County, MD.
Responsibilities
As a key member of our team, you will be responsible for providing high-quality care to our primary and palliative patients.
This will involve:
• Assessing patient health through physical examinations and medical histories
• Creating personalized care plans to meet the unique needs of each patient
• Effectively communicating with office staff and clinical operations
• Providing patient-centered care, including developing diagnoses and treatment plans, ordering diagnostic testing, and prescribing medications
Requirements
We are looking for a qualified Advanced Provider with:
• Board certification in the state of Maryland
• Strong clinical skills and the ability to accurately and confidently treat, assess, and diagnose
• Positive demeanor and strong people skills
• Ability to work independently and problem-solve effectively
• 1-2 years of primary care experience, with geriatric patients preferred
