Transition Coordinator Job Description

We are seeking a skilled Transition Coordinator to join our team at New Story - Greenwich Education Group. The successful candidate will be responsible for coordinating and managing the Career and Transition Services Program, ensuring seamless transitions for students.

Key Responsibilities:
  • Develop and implement career and transition services, including individualized plans, job placements, and transportation coordination.
  • Establish and maintain working relationships with organizations and internal staff to promote and implement student job site placement.
  • Create in-house skills development programs to support vocational placement.
  • Provide instruction to students on vocational placement and motivate them using individualized instructional methods.
  • Monitor progress and maintain data related to deadlines for IEPs as it relates to career and transition services.
  • Communicate regularly with students and guardians regarding transition plans.
  • Utilize non-violent crisis intervention techniques and physically assist as needed.
  • Attend meetings with school districts and multidisciplinary teams.
  • Transport students in company vehicles for transition program implementation.
Requirements:
  • Excellent communication and organizational skills.
  • Ability to work with diverse populations and build strong relationships.
  • Knowledge of vocational rehabilitation principles and practices.
  • Ability to maintain confidentiality and handle sensitive information.
