Physician - Skilled Nursing Facilities

We are seeking a Board Certified or Eligible Internist, Family Practitioner, Geriatrician, or Hospitalist to join our team as a SNF Physician in the Continuum Complexivist department.
As a SNF Physician, you will work closely with an experienced, cohesive interdisciplinary team in our core Skilled Nursing facilities (SNF), and virtually, in our Medicare Home Health Physician program.
Our team includes nurses, therapists, and social workers that provide after-hospital care in the SNF setting. We emphasize transitions of care from the hospital to the skilled nursing setting and from a skilled nursing facility to home or other settings of custodial care.
Responsibilities- Care for a spectrum of medical complexities, ranging from straight-forward, elective orthopedic surgery patients to those with multiple, complex medical problems at each facility.
- Work primarily in Vancouver, WA, and be comfortable working regionally to support patients in East Portland, OR, and Longview, WA, on an intermittent basis.
- Support our virtual care model, which includes early, day, and evening shifts, once per week.
- Collaborate with our Transition Nurse Team for escalations of patients transitioning from recent Hospitalist discharges or SNF discharges.
