Physician - Skilled Nursing Facilities

We are seeking a skilled and compassionate physician to join our team as a Skilled Nursing Facility (SNF) physician. As a member of our Continuum Complexivist Department, you will work closely with an experienced interdisciplinary team to provide high-quality care to patients in our core SNF settings.
Our team emphasizes transitions of care from the hospital to the skilled nursing setting and from a skilled nursing facility to home or other settings of custodial care. We provide a team approach, usually with one to two SNF physicians (MD/NP) and a Kaiser Care Coordinator.
Responsibilities- Care for a spectrum of medical complexities, ranging from straight-forward, elective orthopedic surgery patients to those with multiple, complex medical problems at each facility.
- Work closely with an experienced, cohesive interdisciplinary team in our core SNF settings and virtually, in our Medicare Home Health Physician program.
- Support patients in Vancouver, WA, and be comfortable working regionally to support patients in East Portland, OR, and Longview, WA on an intermittent basis.
- Enjoy a predominantly weekday, daytime work schedule, with occasional virtual shifts from home to support our home health field staff and consults.
- Participate in our Transition Nurse Team for escalations of patients transitioning from recent Hospitalist discharges or SNF discharges.
- Board Certified or Eligible Internist, Family Practitioner, Geriatrician, or Hospitalist.
- Experience with digital technologies, such as AI platforms, telehealth, and/or digital health.
- Ability to work in a team environment and communicate effectively with patients, families, and healthcare professionals.
- Strong clinical skills and ability to manage complex medical cases.
