In collaboration with patients, their families, and other healthcare team members, the Transitional Care Registered Nurse is accountable for assessing, planning, implementing, and coordinating treatment plans to achieve the objectives of the nursing department.
You will play a crucial role within the interdependent team, responsible for delivering positive patient and departmental outcomes.
Key Responsibilities:
- Maintain overall accountability for assigned patients.
- Manage the efficient flow of patient care, including screening, preparation for examinations, providing general nursing care, and coordinating admissions, testing, and discharges.
- Maintain and review patient records, including posting test results and examination findings.
- Administer medications as per physician orders.
- Educate patients and families on medications and treatment plans.
- Respond to and refer incoming calls, including phone triage when necessary.
Qualifications:
- Must be a graduate from an accredited nursing program: Associate's Degree in Nursing or higher is required.
- Must hold a valid Registered Nurse license in the state of practice and/or possess an active Compact/Multi-State RN license.
- A current American Heart Association (AHA) or American Red Cross Basic Life Support (BLS) Health Care Provider certification is required.
