At CenterWell Home Health, we're dedicated to delivering exceptional care to our patients. As a Home Health Physical Therapist Assistant, you'll play a vital role in helping our patients recover and regain their independence.

Key Responsibilities:
  • Administer physical therapy treatment plans developed by physical therapists.
  • Provide non-complex active and passive manual therapeutic exercises, modalities, and treatments to patients.
  • Accurately document patient records and submit clinical paperwork on time.
  • Participate in educational meetings and contribute to department in-services.
  • Notify physical therapists or Executive Director of equipment failures, supply issues, staff changes, and other departmental concerns.
  • Schedule clients and perform various office tasks.
  • Apply infection control and universal precautions in patient care activities.
  • Accept clinical assignments consistent with education and competence to care for patients.
  • Promote compliance with fiscal intermediaries and third-party payors through education, coaching, and assistance.
Requirements:
  • Associate degree in Physical Therapy from an American Physical Therapy Association.
  • Current state license to practice Physical Therapy.
  • Valid driver's license, auto insurance, and reliable transportation.
  • Proof of current CPR certification.
  • Minimum two years experience as a Physical Therapy Assistant.
  • Comprehensive knowledge of geriatric population changes.
  • Self-directed, enthusiastic, and accepts constructive feedback.
  • Good verbal and written communication and organization skills.
  • Professional image and attitude for patients, families, clients, coworkers, and others.
  • Maintains clinical competency and meets mandatory continuing education requirements.
  • Ability to work a flexible schedule and travel locally.
  • Able to maintain confidentiality.
  • Maintains current licensure certifications and meets mandatory continuing education requirements.
  • Fluent English language skills.
  • Good and regular attendance.
