Job Summary

We are seeking a highly skilled and compassionate Family Medicine Practitioner to join our team at Family & Community Services, Inc. As a key member of our healthcare team, you will provide high-quality primary medical and behavioral health services to adults, children, and families in a holistic manner.

Key Responsibilities
  • Examine, diagnose, and treat illnesses and injuries of patients throughout the lifespan.
  • Provide well-child, well-woman, and well-man health maintenance exams to new and established patients.
  • Order appropriate screening and diagnostic lab and radiographic exams.
  • Interpret results of physical exams and screening/diagnostic tests to implement appropriate care plans for the patient.
  • Prescribe medicines, therapeutic agents, and treatment modalities as required by patient needs and in accordance with licensed prescriptive authority.
  • Support health promotion, health education, health empowerment, and disease prevention interventions for all patients.
  • Provide age-appropriate care to all patients.
  • Promote patient and family-centered care through a collaborative team approach.
  • Participate in established call groups as necessary.
  • Refer patients to specialty practices as appropriate and needed.
Requirements
  • Formal MSN-NP/PA training and certification in the state of Ohio to practice medicine.
  • Knowledge of principles and practices of preventative medicine.
  • Knowledge of state and federal laws pertaining to medicine and public health.
  • Knowledge of the structure and function of community health centers.
  • Skill in communicating effectively with patients and families to include culturally sensitive care.
  • Skill in establishing and maintaining effective working relationships with other employees, patients, and the general public.
  • Commitment to Family & Community Services, Inc. mission, vision, and values.
Preferred Qualifications
  • Board certification from ANCC/NCCPA or other valid NP/PA accreditation body with a focus in family medicine (all ages).
  • Licensed in the state of Ohio.
  • Current DEA Registration.
  • Bilingual a plus.
