Job SummaryWe are seeking a skilled and compassionate Private Duty Nurse LPN to join our team. As a Private Duty Nurse, you will provide one-on-one care to our patients in the comfort of their own homes. You will work closely with our patients, their families, and our healthcare team to develop and implement personalized care plans that meet their unique needs.
Responsibilities- Implement physicians' prescribed plans of care
- Conduct patient assessments and coordinate care
- Administer prescribed medication, treatments, and therapies
- Document care provided in our system of record
- Provide health education and training to patients and their families
- Accept and write verbal orders from physicians as required
- Follow up with patients, families, and healthcare team on patient problems, concerns, and changes in patient condition
- Active, unencumbered license (RN/LPN/LVN) in the state of practice
- Current CPR certification from American Red Cross, American Heart Association, or BLS
- TB skin test (current within last 12 months)
- Six months prior hands-on nursing experience preferred
- Reliable transportation
