Job Summary

Key Responsibilities
  • Perform case management duties, including connecting individuals to medical care, insurance enrollment, and other social services.
  • Conduct regular client reassessments and document progress.
  • Develop and provide supportive referrals for clients in need of housing, financial, nutrition, and psychosocial support.
  • Provide transportation, interpretation, and access assistance to clients.
  • Fulfill charting and data collection duties, including training on electronic health records.
  • Conduct home visits and transport clients to appointments as needed.
  • Attend community meetings to ensure client needs are represented.
  • Establish and maintain relationships with staff, clients, and community partners.
Requirements
  • Bachelor's or associate's degree in human services or related field desired or 2 years of experience in relevant human services required.
  • Case management or care coordination experience required.
  • Demonstrated ability to provide bilingual case management in priority populations is strongly preferred.
  • Experience working with HIV/AIDS populations and substance abuse/addictions is strongly desired.
  • Ability to multi-task, work independently, and in a team environment required.
  • Excellent organizational, written, and oral communication skills required.
  • Proficiency in Microsoft Platforms, including Word, Teams, and Outlook required.
  • Valid driver's license and own means of transportation required.
Preferred Qualifications
  • Resident of or familiarity with Brockton area desired.
  • Experience working with harm reduction theories desired.
  • Persons with lived experience, BIPOC, and LGBTQIA+ individuals strongly encouraged to apply.
