Job SummaryWe are seeking a skilled Physical Therapist to join our CarePartners Home Health team. As a Primary Physical Therapist, you will manage an assigned caseload of patients, providing therapy and clinical case management to improve patient outcomes and manage resources effectively.
Key Responsibilities- Conduct comprehensive assessments, examinations, and evaluations to develop and modify plans of care in conjunction with patients, caregivers, and physicians.
- Provide treatment visits and discharge planning to facilitate patient engagement and achievement of goals, clinical outcomes, and positive patient experience.
- Communicate handoff information and coordinate with Physical Therapist Assistants, other therapists, and care team members to support patient care throughout home health and the continuum of care.
- Current North Carolina license to practice Physical Therapy.
- Driver's license required.
- BLS/CPR Certification.
- One to two years of related experience.
- Preferred experience in clinical affiliations with a wide variety of acute care procedures, especially orthopedics, neurology, cardiopulmonary, and care of burns/open wounds.
