The estimated salary for this role is $44.00 per hour, plus shift differentials where applicable. This competitive compensation package reflects the value we place on our employees' skills and experience.
Job Description
We are seeking a skilled Nurse Care Coordinator Leader to join our team. In this role, you will oversee and provide direct care to clients while managing a team of RN Case Managers. Your primary responsibilities will include:
- Planning and evaluating care for assigned clients
- Collaborating with other healthcare professionals to ensure seamless care delivery
- Recruiting, training, and supervising RN Case Managers to achieve excellent patient outcomes
Required Skills and Qualifications
To be successful in this role, you must possess:
- A current, unrestricted RN license in the state(s) of practice
- Minimum of two (2) years nursing experience, including one (1) year in home care or closely related field
- One (1) year of supervisory and/or case management experience preferred
- Excellent communication and leadership skills
