Job Title: Advanced Practice Nurse

North Ottawa Community Health System is seeking a highly skilled Advanced Practice Nurse to join our team. As a key member of our healthcare team, you will be responsible for providing high-quality patient care and contributing to the development of our clinical protocols.

Responsibilities:
  • Perform comprehensive assessments and histories, including subjective and objective data obtained from all relevant sources.
  • Collaborate with the medical director to develop patient care plans and make home visits as needed.
  • Make rounds in the Hospice Residence/Hospital as required and complete, review, and sign documents timely to meet regulatory requirements.
  • Assist in the development and review of clinical protocols and promote open communication across the continuum.
  • Act as a liaison to physicians in the community and promote Hospice and Palliative Care through personal educational contact with health professionals and the community.
Requirements:
  • RN with Michigan license, Masters degree in Nursing required with Advanced Practice Certification.
  • Current BLS certification and valid Michigan driver's license.
  • Experience in Internal Medicine or family Practice required, Hospice and/or geriatric experience preferred.
  • Certification in Hospice and Palliative Care preferred or willing to obtain in 3 years.
  • Ability to manage several job tasks simultaneously and function effectively under high stress situations.
Working Conditions:
  • May work in unpredictable environment, including patient home settings with diverse social, cultural, and economic ranges.
  • Required to drive in all types of weather conditions.
Critical Demands:
  • Able to lift 50 pounds.
  • Able to climb multiple stairs with medical evaluation equipment.
  • Able to sit/drive/stand for extended periods of time.
  • Able to deal effectively with potentially combative, confused patients in the home setting.
