Medical Health Risk Assessment Specialist

We are seeking a skilled Medical Health Risk Assessment Specialist to join our team at Matrix Medical Network. In this role, you will conduct comprehensive health assessments for at-risk individuals in their homes or other community environments.
About the Position:
As a Medical Health Risk Assessment Specialist, you will review and observe patients' current health, medical history, medication adherence, social environment, and other risks. This provides valuable insight into their overall health and well-being that can be difficult to capture during routine office visits.
This position requires strong communication skills, both written and verbal, as you will work collaboratively with physicians, case managers, social workers, family members, key caregivers, and other healthcare professionals to provide comprehensive care.

Responsibilities:
- Conduct comprehensive health assessments, including medical history, diagnosis, treatment, health education, physician referrals, case management referrals, follow-up, and clear documentation according to Matrix guidelines and protocols
- Work collaboratively with healthcare professionals to provide comprehensive care
- Collaborate with Primary Care Physicians (PCPs) on patient education and provide follow-up
- Provide services in various venues, including home visits, skilled facility visits, and telehealth visits
Requirements:
To be successful in this role, you must possess:
- A Master's Degree in Nursing or a related field, or commensurate experience and satisfactory completion of NP licensure
- Current RN and NP/PA licensure in the state of practice, including prescription authority or the ability to obtain prescriptive authority
- Board certification by the NCCPA, AANP, ANCC, or AACCN in a Matrix-approved specialty
- Strong computer skills and familiarity with EMR software
- Excellent verbal and written communication skills
- A positive and outgoing personality, with a strong ability to work collaboratively as part of a team
Salary and Benefits:
The estimated salary range for this position is $2,300 - $11,500 per month, based on an average of 20-100 completed visits per month, with pay of $115 per completed assessment.
PRN/Part-Time providers are non-benefited, except for 401(k) and limited voluntary benefits. Mileage reimbursement is also available.
