Hospice RN Job Description

This role involves providing direct patient care, working closely with the interdisciplinary team to develop and implement patient plans, and participating in coordination of hospice services. As a Hospice RN, you will have the opportunity to make a meaningful difference in the lives of patients and their families.

Responsibilities
  1. Assess patients' physical, emotional, and spiritual needs and develop individualized care plans.
  2. Implement care plans, including administering medications, providing wound care, and offering emotional support.
  3. Communicate effectively with patients, families, and healthcare teams to ensure seamless care transitions.
Qualifications
  • BLS certification.
  • Current RN licensure.
  • Experience working in hospice or palliative care preferred.
