About the Job
CareOregon is a nonprofit organization providing medical care exclusively in the home. We are seeking a Licensed Practical Nurse/Intake and Triage - Hospice to join our team. This role will provide clinical triage support to patients, families, and caregivers while reviewing clinical documents and collaborating with clinicians.

Responsibilities
This position will include providing telephonic triage support, managing incoming phone calls for hospice, and reviewing referrals for appropriateness for hospice services. The successful candidate will also assist in reviewing and assigning RN case managers and the hospice team to new referrals. Additionally, they will gather and review medical records, document information accurately, and perform other duties as needed.

Requirements
We are looking for an experienced Licensed Practical Nurse with at least 1 year of experience in hospice, palliative care, or oncology. A graduate of an accredited LPN training program and current unencumbered LPN license in the State of Oregon are required. Experience providing triage support is preferred. Knowledge of Federal, State, and Local regulations, Hospice Medicare Conditions of Participation, and HIPAA privacy rules and regulations is essential.

Benefits
This hybrid position offers a competitive salary range of $68,000-$83,000 per year, with a 5% bonus target. Full benefits, including health insurance, retirement savings, and paid time off, are also provided.
