About the Role:
  • Provide direct patient care, implementing plans of care initiated by registered nurses.
  • Collect assessment data, perform observations, and report findings to RNs and physicians.
  • Participate in coordination of home health services, identifying needs for other disciplines.
  • Use equipment and supplies effectively and efficiently.
  • Contribute to personal and professional growth and development.
Requirements:
  • Licensed Practical Nurse (LPN) licensure in Oregon.
  • At least 1 year of nursing experience.
  • Able to work independently or as part of a dynamic team.
  • Balance clinical excellence with empathy and a zest for life.
  • Hold current BLS certification from AHA.
  • Reliable vehicle for travel to patient visits.
