About Us
Princeton Transitional Care & Assisted Living is a partnership between Signature HealthCARE and Ballad Health, bringing a new level of healthcare services to the Tri-Cities area.
The new Princeton Transitional Care will provide a new home for skilled-nursing care and add 60 assisted living apartments, 47 short-term skilled-nursing and rehab private suites.

Signature HealthCARE - Excelerated Care
Our designated short-stay unit, Signature HealthCARE - Excelerated Care, specializes in rehabilitation and clinical services for those recovering from surgery, illness, injury, or a recent hospital stay.
This unit is comprised of dedicated staff, private suites, select dining options, and more to assist patients on their road to recovery.
Our Services
Signature HealthCARE offers integrated services across the continuum of care, including skilled nursing, rehabilitation, assisted living, memory care, home health, cognitive care, and telemedicine.
Many of our skilled nursing facilities have achieved a 4 or 5-star overall rating from the Centers for Medicare & Medicaid Services.

As a Weekend Licensed Practical Nurse, you will deliver exceptional nursing services to our patients and residents, maintaining the highest degree of quality and compassionate care.
Responsibilities
- Deliver exceptional nursing services to our patients and residents, maintaining the highest degree of quality and compassionate care.
- Supervise day-to-day activities performed by assigned nursing assistants.
- Operate in accordance with Federal, State, and Local Standards, guidelines, and regulations, and as required by the Director of Nursing.
- Confirm that all nursing personnel assigned to you comply with the written policies and procedures established by the facility.
Requirements
- Possess an active LPN license in the state of residence.
- Current/active CPR Certification.
- Strong attention to detail and accuracy, excellent organizational skills, with the ability to prioritize, coordinate, and simultaneously maintain multiple projects with a high level of quality and productivity.
- Excellent technical, assessment, and documentation skills, with the ability to maintain confidentiality.
