Job Summary:

As an Outreach Specialist with Covenant Health (Tennessee), you will play a vital role in providing care monitoring and in-person support services to assigned consumers. This position requires a strong understanding of community-based services and the ability to engage service participants in services within the healthcare system and the general community.

Key Responsibilities:

• Meet with the interdisciplinary team weekly to review treatment needs of consumers.
• Contribute to the development of comprehensive care plans for designated service participants.
• Work with service participants and their families to understand, acquire, and maintain needed healthcare services and develop a healthy lifestyle.
• Assess life domain needs of consumers, including living arrangements, financial, vocational/educational, health, social support, leisure/recreational, and emotional/mental health.
• Assist service participants in developing skills related to independent living and navigating the service delivery system.
• Provide ongoing linkage, monitoring, planning, and evaluation, and advocacy for team consumers.

Requirements:

• Bachelor's Degree in a health-related field of counseling, psychology, or social work, or must be licensed as a nurse (LPN or RN).
• Minimum of one year of relevant experience if bachelor's degree is not in counseling, psychology, or social work or if not licensed as a registered nurse.
• Valid Tennessee driver's license Class D and state-mandated minimum insurance coverage.
• Driving record must meet Covenant Health minimum standards at the date of hire and throughout employment tenure.
