We are seeking a highly organized and detail-oriented Care Coordinator to join our team at Arcadia Home Care and Staffing. As a Care Coordinator, you will be responsible for coordinating client schedules and ensuring that client care plans are followed.

Key Responsibilities:
  • Schedule Home Care Aides as directed by client care plans
  • Create and manage client schedules, including making changes due to client requests, illness, vacation, or leaves of absence
  • Provide alternate coverage to ensure client care plans are followed and client services are not interrupted
  • Track and report daily scheduling metrics and communicate client scheduling trends to management
  • Process patient authorizations and communicate with central admissions
  • Ensure accurate HR system records and maintain confidentiality of sensitive information
  • Ensure the appearance of the branch's open environment is professional and maintain regular, predictable attendance
Requirements:
  • High school diploma or equivalent; college degree preferred
  • 1 year of home care scheduling and administrative experience
  • Organizational and multitasking skills
  • Computer proficiency in MS Office
  • Ability to work well with others in a team-oriented environment
