Licensed Clinical Social Worker

We are seeking a highly skilled Licensed Clinical Social Worker to join our team at Rehab Without Walls Neuro Rehabilitation. As a key member of our interdisciplinary team, you will provide comprehensive social work services to patients in their homes and communities, promoting optimal health and well-being.
Responsibilities- Provide direction, supervision, and evaluation of medical social services to patients in the home, ensuring compliance with state practice acts and company policies.
- Instruct, treat, observe, and evaluate patients with significant social and emotional problems affecting their health status.
- Assist physicians and other healthcare team members in understanding social and emotional factors related to patients' health problems.
- Participate in the development and periodic re-evaluation of the physician's Plan of Treatment for patients requiring social work services.
- Utilize special skills and judgment to resolve social and emotional responses connected with illnesses.
- Observe, record, and report changes in patients' condition and response to treatment to the Clinical Manager and physician.
- Participate in discharge planning and provide ongoing assessment of patient and family needs and responses to teaching.
- Supervise, instruct, and evaluate the performance of the Social Work Assistant (BSW) to ensure compliance with company, government, and professional standards.
- Maintain and submit documentation as required by the company and/or facility, including case conferences, patient/physician/community contacts, visit reports, progress notes, and confers with other healthcare disciplines to provide optimum patient care.
- Attend, participate in, and/or conduct internal staff development programs, obtain continuing education as required by company policies and regulations, and maintain the highest standards of professional conduct in relation to confidential information.
- Bachelor's Degree required; Master's Degree preferred from a school of social work accredited by the Council on Social Work Education with six months' social work experience in a healthcare setting.
- Current licensure/certification in the state(s) of practice if required.
- CPR certification required.
- Successful completion of any/all company required pre-employment competency testing and health clearance prior to assignment and periodically as required by company policy and state regulations.
- Good organization and communication skills and ability to function with minimal supervision also required.
