As a Physical Therapist with VNA Health Group, you will have the opportunity to make a meaningful difference in the lives of our patients and their families.
Responsibilities:- Assess and evaluate patients for physical therapy needs and develop individualized plans of action in collaboration with physicians.
- Provide physical therapy evaluation and treatment to patients in their homes or alternate community settings.
- Document all changes in patient condition and update care plans and outcomes accordingly.
- Identify counseling needs and plan appropriate teaching strategies.
- Monitor quality and appropriateness of services provided.
- Physical Therapy Licensure in NJ required.
- 2-3 years of prior clinical experience and community-based experience preferred.
- Strong computer skills.
- Strong communication and interpersonal skills.
- Valid NJ driver's license.
