As an Oncology Clinical Pharmacy Expert, you will play a pivotal role in managing pharmacotherapy for adult patients with complex malignancies in an outpatient setting. You will collaborate closely with a multidisciplinary oncology team to ensure optimal patient outcomes throughout their treatment journey.
Key Responsibilities:
- Oversee and manage pharmacotherapy for patients with gastrointestinal, sarcoma, and head and neck cancers.
- Engage in direct patient care through in-person and telehealth consultations, providing education and support for oral anticancer therapies.
- Conduct medication reconciliation and monitor patients for therapy initiation and side effects.
- Serve as a resource for the oncology team regarding drug information, toxicity management, and appropriate dosing.
- Educate and mentor pharmacy residents and students, fostering the next generation of pharmacy professionals.
- Participate in departmental committees to contribute to formulary management and policy development.
Qualifications:
- Current pharmacist licensure in the relevant state.
- Doctor of Pharmacy (Pharm.D.) degree or equivalent experience.
- Completion of a PGY1 residency and a PGY2 specialty residency in oncology preferred.
- Board of Pharmacy Specialties (BPS) certification required within two years of employment.
- Intravenous (IV) Certification and Basic Life Support (BLS) certification required within specified timeframes.
