About the Role

Concerto Renal Services is a leading provider of nursing home dialysis services, dedicated to delivering exceptional care to patients with end-stage renal disease. We are seeking a highly skilled Dialysis Nurse to join our team.

Key Responsibilities
  • Supervise Dialysis Care: Ensure that dialysis care is provided in accordance with policy, procedure, and training, and in compliance with state and federal regulations.
  • Collaborate with Healthcare Team: Work collaboratively with members of the hemodialysis healthcare team to provide comprehensive care to patients.
  • Provide Education and Support: Provide continuous education to patients regarding their renal disease, vascular access, dialysis therapy, and other health-related conditions.
  • Delegate Tasks: Delegate tasks to dialysis technicians to ensure safe and effective delivery of dialysis therapy as prescribed.
  • Monitor and Document: Monitor hemodialysis care and ensure accurate and timely documentation by dialysis technicians.
  • Assess Patient Response: Assess patients' response to hemodialysis therapy and make appropriate adjustments to the treatment plan as prescribed.
  • Ensure Compliance: Ensure that lab specimens are properly obtained and packaged according to Department of Transportation (DOT) policies and procedures.
  • Communicate with Healthcare Team: Collaborate and communicate with other members of the hemodialysis healthcare team to interpret, adjust, and coordinate care provided to patients.
  • Document Patient Data: Ensure that all relevant data, including physician orders, lab results, medication administration, admission/discharge data, treatment parameters, and patient condition, are documented appropriately and entered into the electronic medical record.
  • Review Lab Results: Review lab results timely and report critical data to the physician.
  • Ensure Shift Operations: Maintain overall shift operation in a safe, efficient, and effective manner.
Requirements
  • Education: Graduate of an accredited school of Nursing.
  • License: Current state-appropriate license.
  • Experience: Minimum of one (1) year of nursing experience, with dialysis experience required.
  • Certifications: CPR certified.
