Amerita is seeking a skilled and compassionate registered nurse to join our team as a Home Infusion RN - Patient Care Coordinator. In this role, you will provide high-quality patient care and coordination services to our specialty infusion patients in their homes.

We are dedicated to providing complex pharmaceutical products and clinical services to patients outside of the hospital setting. With our focus on quality care and customer satisfaction, we have built strong relationships with our patients, families, and healthcare partners.

Your Responsibilities
The Home Infusion RN - Patient Care Coordinator will be responsible for:
  • Developing and implementing individualized care plans for patients
  • Collaborating with multidisciplinary teams to ensure seamless patient care
  • Providing education and support to patients and caregivers
  • Monitoring patient progress and making recommendations for changes to treatment plans as needed

Requirements
To succeed in this role, you must have:
  • A graduate degree from an accredited school of nursing; Bachelor of Science in Nursing preferred
  • A valid nursing license in state(s) of practice
  • A minimum of one year of experience as a Registered Nurse
  • Previous infusion and home health care experience preferred
  • Current CPR certification
  • Flexible schedule with the availability to work a variety of shift models
  • Effective verbal and written communication skills
