Key Responsibilities:
- Conduct in-home assessments to identify clients' needs and develop comprehensive care plans
- Collaborate with healthcare providers to ensure clients receive high-quality care

- Monitor clients' progress and adjust care plans as needed
- Provide advocacy and support to clients and their families

Requirements:
- Nursing degree or equivalent experience
- Current licensure as a registered nurse
