Registered Nurse (RN) - Hospice Job Description

We are seeking a Registered Nurse (RN) with a passion for providing high-quality hospice care to join our dynamic team. As an RN, you will be responsible for delivering comprehensive care to patients in their homes, working collaboratively with our interdisciplinary team to develop personalized care plans and ensuring seamless transitions between care settings.

Responsibilities:
  • Deliver high-quality, patient-centered care that meets the unique needs and preferences of each patient and family
  • Conduct thorough assessments and develop individualized care plans in collaboration with the patient's primary physician and other healthcare providers
  • Maintain accurate and up-to-date records of patient interactions, progress notes, and other relevant documentation
  • Participate in multidisciplinary meetings and conferences to discuss patient cases, coordinate care, and address complex issues
Requirements:
  • A current license as a Registered Nurse in NJ
  • A minimum of one year of recent, verifiable clinical experience
  • Prior home care experience strongly preferred, but not required
  • Graduation from an accredited and approved nursing program
  • Strong communication and interpersonal skills, with ability to build rapport with patients, families, and healthcare colleagues
