We are looking for Home Health Aide contractors who resonate with our core values. Your responsibilities will include providing essential healthcare services in the client's home for the designated care period. The patient is approved for up to 50 hours of care weekly, allowing for flexible scheduling and the need for multiple caregivers. No specialized certifications are required for this position.

Key Responsibilities:

  • Follow the care plan to ensure safe and effective client care.
  • Assist with personal hygiene and maintain a healthy, secure living environment.
  • Plan and prepare nutritious meals.
  • Support approved client mobility.
  • Encourage client independence according to the care plan.
  • Enhance client mental engagement through stimulating activities.
  • Provide emotional and psychological support while upholding client trust and confidentiality.
  • Monitor and report any changes in client health or home conditions to the supervisor.
  • Perform routine housekeeping tasks to ensure a safe and comfortable environment for clients.
  • Document progress notes promptly.
  • Coordinate weekly visit schedules with other staff members.
  • Collaborate with community agency personnel involved in client care.
  • Attend mandatory training sessions as required by state regulations.
  • Communicate with the RN Case Manager regarding client care and status updates.
  • Report any changes in client condition immediately to the RN Case Manager.
  • Operate within the defined scope of practice for Home Aides.

Desired Qualifications:

  • High school diploma or equivalent.
  • Completion of state-required training hours, if applicable.
  • Proficiency in using computer and internet applications for documentation.
  • Strong problem-solving and multitasking abilities.
  • Compassionate and empathetic demeanor.
  • Familiarity with client-related equipment for safe operation.
  • Successful completion of a background check.
  • Professional appearance and demeanor.
  • Good health without risks to clients or colleagues.
  • Understanding of personal care, housekeeping, and meal preparation fundamentals.
  • Commitment to client confidentiality and ethical standards.
