The Inpatient Case Management RN Supervisor plays a crucial role in overseeing and coordinating the inpatient care transitions program. This position ensures that patients navigate through the healthcare continuum effectively, achieving optimal clinical and financial outcomes. The supervisor will engage in the development and execution of integrated care transition strategies across various care settings. Collaboration with a multidisciplinary team is essential, including partnerships with ambulatory services, post-acute providers, and health plans.
Key Responsibilities:
- Lead and mentor the case management team, exemplifying professional practices aligned with organizational objectives and standards.
- Manage the departmental budget and ensure compliance with federal, state, and regulatory standards.
- Facilitate effective communication and collaboration among team members and other healthcare professionals.
Qualifications:
- A minimum of 1 year of leadership experience.
- At least 3 years of experience in Acute Care Case Management.
- Bachelor's degree in Nursing is required; a Master's degree is preferred.
- Current Registered Nurse Licensure.
- Specialty certification must be obtained within one year of eligibility (ACM, CCM, CCCTM, CMAC, or CMGT-BC).
- BLS certification is required within 90 days of hire.

This team operates in a collaborative environment focused on patient-centered care, managing an average of 15 cases daily with a strong emphasis on urgency and responsiveness.
