Nurse Manager Assistant Case Management

We are seeking a highly skilled and experienced Assistant Nurse Manager Case Management to join our team at the Commonwealth of Virginia. As a key member of our healthcare team, you will play a vital role in supporting the nurse manager in administrative duties and providing leadership to ensure high-quality patient care.
Key Responsibilities:- Provide first-line supervision to RN Case Managers and support staff in the inpatient setting.
- Perform case manager duties in accordance with established policies, procedures, and standards of practice.
- Conduct initial assessments on all patients and initiate discharge planning from day one of their stay.
- Communicate with payers to facilitate efficient patient movement through the hospital system.
- Complete level of care assessments to ensure patients receive the appropriate level of care and transition smoothly through their stay.
- Work closely with admitting physicians to ensure accurate documentation of clinical information.
- Coordinate discharges home and facilitate transfers to external placement facilities.
- Maintain expert knowledge of Interqual Level of Care criteria.
- Oversee care provided by maintaining high visibility to patients, families, and staff in the clinical area.
- Share accountability with manager for departmental goals achievement, including patient experience, team member engagement, and continuous process improvement.
- Staff management, including recruitment, retention, coaching, training, evaluation, and development, as well as preparation and delegation of work schedules and workflow.
- Participate in basic human resource functions under supervision from manager.
- Maintain a positive and professional work environment.
- Be accountable for outcomes.
- Identify patient readiness for discharge based on outcomes.
- Review regulatory requirements to ensure high-quality, safe, and cost-effective care.
- Identify patients with high-risk concerns for presentation at Clinical High-Risk meeting based on experience, high-risk report, or denials.
- Evaluate and measure outcomes to continuously improve practice and enhance outcomes.
- Maintain records pertaining to staff competency and quality audits.
- Serve as a professional role model, coach, and resource for employees.
- Participate actively in Nursing and/or hospital committees and work groups.
- Assume responsibility for own professional growth.
Requirements:
- Bachelor's of Science in Nursing required.
- 5 years clinical nursing experience, including 3 years case management experience, required.
- Licensed to Practice as a Registered Nurse in the Commonwealth of Virginia.
- American Case Management Association or Commission for Case Manager certification required within 1 year of hire.
Physical Demands:
- Job requires standing for prolonged periods, frequent walking, bending/stooping.
- Proficient communicative, auditory, and visual skills; attention to detail and ability to write legibly.
- Ability to lift/push/pull lbs.
- May be exposed to chemicals, blood/body fluids, and infectious disease.
