Job Summary

We are seeking a Certified Nursing Assistant to provide high-quality patient care in a homecare setting. As a member of our care team, you will work closely with RN Case Managers, Social Workers, and other healthcare professionals to deliver exceptional care to our patients and their families.

Responsibilities
  • Assist patients with activities of daily living, including bathing, dressing, and grooming
  • Provide homemaker assistance, such as light housekeeping and meal preparation
  • Communicate effectively with patients, families, and healthcare professionals to ensure seamless care
  • Document patient care and changes in condition in electronic health records
  • Participate in meetings to plan and evaluate patient services
Requirements
  • High school diploma or equivalent
  • Graduation from an accredited Certified Nursing Assistant program
  • Current, unrestricted Nursing Assistant Certification in state of employment
  • One year of personal care experience, hospice, or home health experience preferred
  • Valid driver's license, auto insurance, and access to a reliable vehicle
