About the Role

Agrace is a nonprofit, community healthcare organization that is nationally recognized as a leader in the hospice industry. We are seeking a skilled Certified Nursing Assistant to join our Inpatient Unit team.

Key Responsibilities
  • Assist patients with activities of daily living, including ambulation, bathing, dressing, and grooming.
  • Provide homemaker assistance, such as light housekeeping and delivering patient supplies.
  • Communicate effectively with patients, families, and healthcare team members to ensure high-quality care.
  • Document patient care and changes in condition in electronic health records.
  • Participate in meetings to plan and evaluate patient/family services.
Requirements
  • High school diploma or equivalent.
  • Graduation from an accredited Certified Nursing Assistant program.
  • Current, unrestricted Nursing Assistant Certification in the state of employment.
  • One year of personal care experience, hospice, or home health experience preferred.
  • Ability to develop, maintain, and apply knowledge of hospice and end-of-life care.
  • Ability to maintain professional boundaries with patients, families, and coworkers.
  • Basic Microsoft Office skills and experience using electronic health records.
