Palliative Care Specialist

The Palliative Care Specialist collaborates with physicians to provide comprehensive care to patients with serious illnesses. This role involves working with patients and their families to develop individualized care plans that address their physical, emotional, and spiritual needs. The Specialist will work closely with the healthcare team to ensure that patients receive timely and effective care, and will provide education and support to patients and their families to help them navigate the healthcare system.

Key Responsibilities:

* Collaborate with physicians to develop and implement individualized care plans for patients with serious illnesses
* Provide education and support to patients and their families to help them navigate the healthcare system
* Work closely with the healthcare team to ensure that patients receive timely and effective care
* Develop and maintain relationships with community resources and healthcare providers to ensure seamless care coordination

Requirements:

* Master's degree in Nursing or related field
* Current licensure as a Registered Nurse in the state of North Dakota or Minnesota
* Certification as a Nurse Practitioner or Advanced Practice Registered Nurse
* 1 year of experience in palliative care or a related field

Preferred Qualifications:

* Experience working with patients with serious illnesses
* Certification in a related field, such as hospice or palliative care
* Bilingual or multilingual skills

Work Environment:

The Palliative Care Specialist will work in a fast-paced, dynamic environment with a team of healthcare professionals. The Specialist will be required to work independently and as part of a team to provide comprehensive care to patients. The Specialist will also be required to communicate effectively with patients, families, and healthcare providers to ensure seamless care coordination.

Physical Demands:

The Palliative Care Specialist will be required to sit for long periods of time, and will also be required to stand, walk, and move around the hospital and clinic settings. The Specialist will also be required to lift, carry, and transfer patients as needed. The Specialist will be required to work in a variety of environments, including hospitals, clinics, and patient homes.

Driving Requirements:

The Palliative Care Specialist will be required to drive occasionally to visit patients in their homes or in other healthcare settings. The Specialist will be required to have a valid driver's license and a clean driving record.

Working Conditions:

The Palliative Care Specialist will work in a variety of environments, including hospitals, clinics, and patient homes. The Specialist will be required to work in a fast-paced, dynamic environment with a team of healthcare professionals. The Specialist will also be required to work independently and as part of a team to provide comprehensive care to patients.
