Primary Care and Addiction Medicine Nurse Practitioner

We are seeking a highly skilled and compassionate Advanced Practice Registered Nurse (APRN) to join our multidisciplinary team at Aspire Health Partners. As a key member of our integrated care continuum, you will play a vital role in providing exceptional medical services to patients with a focus on behavioral health, addiction medicine, and primary care.
Responsibilities- Conduct comprehensive medical assessments to determine appropriate treatment plans and develop individualized treatment plans in collaboration with the patient and the care team.
- Prescribe medications and monitor their effectiveness and side effects, providing medication management and adjustments as needed.
- Collaborate with other healthcare professionals to ensure coordinated and integrated care, including regular follow-ups and monitoring patient progress.
- Maintain accurate and up-to-date medical records and documentation, staying informed about the latest research and advancements in integrated care.
- Valid APRN license in Florida.
- Master's degree in Nursing or Post MS Certificate from an accredited University/College.
- Board Certification as a Family Nurse Practitioner (FNP), Psychiatric Mental Health Nurse Practitioner (PMHNP), or Adult Gerontological Primary Care Nurse Practitioner (AGPC).
- 2 years of experience as a Nurse Practitioner (preferred). Willing to work with motivated new graduates.
- Excellent clinical assessment, diagnostic, and treatment skills, with the ability to work collaboratively in a multidisciplinary team environment.
- Exceptional interpersonal and communication skills, with a patient-centered approach.
- Proficient in using electronic medical record (EMR) systems and other relevant healthcare software.
