Palliative Care Nurse Practitioner Position

Compassus is seeking a skilled Nurse Practitioner to join our team in providing high-quality palliative care to patients and their families. As a Palliative Care Nurse Practitioner, you will be responsible for modeling the Compassus values of Compassion, Integrity, Excellence, Teamwork, and Innovation.
Key Responsibilities- Palliative Care Services
- Provide routine and urgent assessments to patients and their families.
- Develop and implement individualized care plans to meet the unique needs of each patient.
- Collaborate with the interdisciplinary team to ensure comprehensive care.
- Provide education and support to patients and their families to promote optimal health outcomes.
- Communication and Collaboration
- Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
- Collaborate with the Compassus Medical Director and other healthcare professionals to ensure high-quality care.
- Education and Training
- Develop and implement educational programs to promote palliative care knowledge and skills among healthcare professionals.
- Provide training and mentorship to nurse trainees and other healthcare professionals.
- Education and Experience
- Master's degree in Nursing required.
- Minimum of five (5) years nursing experience required; clinical nursing care in oncology, hospice, geriatrics, intensive care preferred.
- Certifications and Licenses
- Active and unencumbered CNS or Nurse Practitioner license in state of employment required.
- Certified as a Nurse Practitioner in Adult, Family, Acute, or Geriatric required.
The demands of this role necessitate a team member to effectively perform essential functions. Adaptations can be made to accommodate team members with disabilities. Regular standing, walking, and manual dexterity are fundamental, along with the ability to lift and move objects up to 50 pounds. Visual acuity requirements include close and distance vision, color and peripheral vision, depth perception, and the ability to adjust focus.
