Palliative Care Nurse Practitioner Position

Compassus is seeking a highly skilled Palliative Care Nurse Practitioner to join our team. As a key member of our interdisciplinary team, you will provide comprehensive care to patients with complex, chronic, and life-threatening conditions. Your expertise will be instrumental in promoting patient comfort, coping, and quality of life.
Key Responsibilities:
- Assess patients' needs and develop individualized care plans to address their physical, emotional, and spiritual needs.
- Collaborate with patients, families, and healthcare teams to ensure seamless care coordination and communication.
- Provide education and support to patients and families on disease management, symptom control, and advance care planning.
- Work closely with the Compassus Medical Director and other healthcare professionals to ensure high-quality patient care.
- Participate in ongoing education and professional development to stay current with best practices in palliative care.
Requirements:
- Master's degree in Nursing required.
- Minimum of five (5) years nursing experience, with clinical experience in oncology, hospice, geriatrics, or intensive care preferred.
- Active and unencumbered CNS or Nurse Practitioner license in state of employment required.
- Certified as a Nurse Practitioner in Adult, Family, Acute, or Geriatric required.
