Job Summary

We are seeking a highly motivated and experienced Intake Coordinator to join our Health Home Care Coordination program. The successful candidate will be responsible for promoting the Health Home program, screening clients for program eligibility, and completing program intakes while adhering to agency and program policies and procedures.

Key Responsibilities
  • Complete screenings for all referrals received to assess eligibility and appropriateness for program enrollment.
  • Ensure client consent for the program and identify and assess an individual's current needs and goals to create an initial care plan.
  • Complete initial billing questionnaire and intake package, and upload all documentation into the electronic health record within 24 hours of completed intake.
  • Document all screening/referral data daily in applicable spreadsheets and conduct community-based engagement efforts to promote Health Home Program services.
  • Collaborate with clinic-based staff, agency staff, and Community Partnerships to ensure seamless care coordination.
Requirements
  • Bachelor's Degree in a health-related field is required; Master's Degree is preferred.
  • Minimum of three years of case management experience in the medical field or social service sector.
  • Understanding of treatment modalities, levels of care, and experience with mental health.
  • Experience with coordinating health-related activities, utilization review, identifying health trends, and analyzing health-related data.
