Job Summary

We are seeking a skilled Physical Therapist to provide high-quality care to patients in their homes. As a Physical Therapist with our team, you will work closely with patients, families, and healthcare professionals to develop and implement personalized plans of care.

Key Responsibilities
  • Provide physical therapy services to patients in their homes, as prescribed by a physician.
  • Conduct initial and ongoing assessments to determine patient's level of functioning and ability to perform activities of daily living.
  • Develop and revise plans of care in consultation with the physician and other care team members.
  • Report patient's reaction to treatment and any changes in their condition to the physician and designated agency representative.
Requirements
  • Licensure as a physical therapist in the state of Florida.
  • Minimum of one (1) year experience in home health care preferred.
