Job Description

Forte Human Capital, Inc. is seeking skilled and compassionate Physical Therapists to provide high-quality care to Veterans under this contract. This full-time position offers a rewarding opportunity to make a positive impact in an outpatient setting.

Key Responsibilities

  • Direct Patient Care (80% of time):
    • Evaluate patient medical records to assess the appropriateness of Physical Therapy (PT) interventions, considering any potential risks or contraindications.
    • Conduct thorough evaluations using standard and non-standard methods to assess patients' medical conditions, motor skills, and rehabilitation potential. Accurately document findings in accordance with departmental and VA standards.
    • Develop personalized treatment plans with clear goals, focusing on subacute and long-term medical rehabilitation. Modify plans as necessary based on patient response and progress.
    • Deliver a full range of PT treatment procedures to address physical and psychosocial needs, ensuring that patients achieve their rehabilitation goals.
    • Regularly discuss patient status, progress, and goals with physicians and interdisciplinary team members to ensure coordinated care.
    • Fit and instruct patients on the use of adaptive equipment and assistive devices, justifying medical necessity for all requests.
    • Follow all infection control and safety standards while ensuring appropriate patient referrals and discharge planning.
  • Administrative Duties (20% of time):
    • Maintain and submit productivity reports and other required documentation in a timely manner.
    • Assist in the development and refinement of PT protocols and guidelines, focusing on subacute and long-term care within the facility.
    • Participate in interdisciplinary team meetings, committee activities, and staff conferences as required by the VA Chief of Physical Therapy.

Qualifications and Experience

  • Must hold a current, full, and unrestricted license to practice as a Physical Therapist in any State, Territory, Commonwealth of the United States, or the District of Columbia.
  • Board Certification: Certified by the American Board of Physical Therapy Specialties (ABPTS).
  • Certifications: Current certification in Basic Life Support (BLS) and Advanced Cardiac Life Support (ACLS) is required.
  • Experience: Minimum of one year of experience in an outpatient physical therapy setting.

Work Schedule

  • Hours: Monday through Friday, 8:00 AM – 4:30 PM, with a 30-minute lunch break.
