Job Summary:

The Clinical Care Coordinator coordinates with healthcare professionals to ensure patients receive medically appropriate and cost-effective care. This role involves assessing patients' needs, documenting assessments, and collaborating with a multidisciplinary team to facilitate timely discharge.

Key Responsibilities:

  • Assess patients' needs upon admission and document assessments in IPOC notes.
  • Contribute to the interdisciplinary plan of care and ensure consistency with discharge plans and patients' continuing care needs.
  • Identify patients with complex discharge planning needs and refer them to the social worker.
  • Assess patients for appropriateness of admission, continued stay, and take action to ensure correct patient status.
  • Communicate with external review agencies regarding concurrent patient review.
  • Discuss appropriateness of admission, continued stay, resource utilization, consultations, estimated length of stay, and discharge plan with attending physicians.
  • Refer cases to the Physician Advisor as necessary.
  • Participate in interdisciplinary patient meetings to coordinate efficient delivery of care.
  • Coordinate activities to ensure patients' appeal rights under the discharge appeal program.
  • Ensure patients receive Important Message from Medicare as required upon admission and discharge.
  • Maintain chart documentation consistent with department and organizational standards.
  • Document reviews and findings on appropriate paperwork and/or software.
  • Demonstrate knowledge of federal and state regulations regarding Medicare, Medicaid, Champus, and other government agencies in healthcare.
  • Maintain clinical competency and current knowledge of regulatory and payer requirements.
  • Participate in the quality improvement process for the Case Management Department.
  • Attend Department and Team staff meetings.

Qualifications:

Education:
Graduate of an accredited School of Nursing with current Alabama licensure.

Experience:
Three years of clinical experience preferred. Comparable combination of education and experience will be considered. Previous utilization management, discharge planning, or hospital-based social service experience is preferred.

Additional Skills/Abilities:
Excellent communication skills with the ability to work in a fast-paced environment, prioritizing and changing tasks frequently and quickly.
