Job Overview

We are seeking a skilled Nursing Care Coordinator to join our team at Prisma Health. In this role, you will provide exceptional patient care and coordination of services to ensure seamless transitions throughout the healthcare continuum.

Responsibilities

Develop and implement individualized plans of care in collaboration with patients, families, providers, and interdisciplinary teams.
• Conduct thorough nursing assessments to identify patient priorities and needs.
• Validate priorities with healthcare team members and document accordingly.
• Develop written assignments and delegate tasks as appropriate to optimize care delivery.
• Monitor quality of care and ensure appropriate surveillance of assigned patients.
• Coordinate and organize tasks to meet patient and unit priorities while incorporating cost containment principles into nursing care.
• Assume responsibility for patient safety by maintaining a safe environment and adhering to patient care standards.

Requirements

To be successful in this role, you will need an Associate's Degree in Nursing and current licensure/registration/certifications. We also require one year of registered nurse experience or successful completion of the Prisma Health Nurse Residency Program or a comparable residency from another organization.

Estimated Salary Range: $64,500 - $81,900 per year
