Key Responsibilities- Document and provide care for assigned patients
- Implement a home health plan that meets each patient's specific needs under the direction of the RN Case Manager
- Comply with JCAHO, federal, and state regulations, reimbursement guidelines, and agency policies and procedures
- Completion of a practical nursing program
- Licensed in the state where practicing
- BLS certification
- Driver's license
- LIC LPN
- One year of experience in Home Health
